Remote Patient Monitoring
Working together to make a difference one person at a time.
Remote Patient Monitoring (RPM) is an effective method of monitoring patients
with certain diagnosis seven (7) days a week. The RPM team in collaboration
with our nursing team identify potential issues before they become a problem
and initiate additional same day skill nursing visits as needed.
Monitoring is done through iPad, Bluetooth, in- home scales, pulse oximeter
and telephone devices. Our Registered Nurse (RN) staffed call center monitor
and educate patients and caregiver on vital changes in blood pressure,
pulse, weight, nutrition, medication that may require immediate attention
and when to contact our team for a same day visit. Our clinical team will
facilitate/initiate contact with your physician related to identify issues
that require medical intervention. This service prevents re-hospitalizations
and allows patients to stay at home knowing that they have the oversight
of a nurse on a daily basis.
