Private Duty

St. Joseph Health Services Home Health Network Private Duty Services helps seniors and others with disabling conditions to get the support they need at home.  Our Caregivers help with the everyday tasks that help individuals remain safe and independent in their homes.

  

Services Offered

Private Duty services are customized to meet your individual needs.  Our services include:

 

  • Bath visits
  • Hourly care (day and night shifts)
  • Sleepovers (providing safety and peace of mind)
  • RN Medication Management
  • Travel escorts
  • Sitter Care Services (Hospital & Skilled Nursing Facility)

We Can Assist With:

  • Skilled care
  • Activities of Daily Living
  • Personal care
  • Meal preparation
  • Medication reminders
  • Companionship
  • Comfort care
  • Transportation
  • Shopping / Errands
  • Laundry
  • Light housekeeping
  • Respite care 

And Help Manage:

  • Individuals with all types of Dementia including Alzheimer’s
  • End-of-life (Hospice) and Palliative Care clients
  • Post-surgery and Rehab clients
  • People who are no longer able to fully care for themselves

Eligibility

Our Private Duty Team

Our compassionate and highly skilled team is dedicated to improving the health of individuals and families in our community.  We work together to make a difference.  Our team includes:

 

  • Registered Nurses
    Our RN Clinical Supervisors provide clinical oversight to clients and other team members, create individualized care plans based on the need(s) and reassess clients and their needs on an ongoing basis.
     
  • Geriatric Care Manager
    A Registered Nurse who specializes in aging and eldercare will develop and help to monitor a program tailored to meet the needs of your family.
     
  • Licensed Vocational Nurses
    Provide skilled nursing care to clients who require a higher level of care, i.e. clients with G-tubes, Catheters, Trachs, Ventilators or who need Medication Administration / Management.
     
  • Certified Home Health Aides, Certified Nursing Assistants and Personal Care Assistants
    These three disciplines assist clients with their activities of daily living, i.e. personal care, meal preparation, medication reminder, light housekeeping and errands.
     
  • Sitters
    Assist clients at bedside in a Skilled Nursing Facility or Hospital, providing peace of mind to the client and family.  They can assist with bathing, feeding and notifying the nurse when there is a change in the client’s condition or pain level.
