Private Duty Geriatric Care Management

Our Geriatric Care Management team is here to help families who are caring for elderly or disabled loved ones. Our Care Managers are Registered Nurses, Medical Social Workers and Therapists who are educated, trained, experienced, licensed health care and social service professionals. All St. Joseph Private Duty team members are supervised by our RN Geriatric Care Manager as part of our home care service. We also provide enhanced Geriatric Care Management to our existing clients and to community residents at an affordable cost. Some clients may qualify for short-term subsidy for both Private Duty care and Geriatric Care Management.

  • Assistance provided for those moving to retirement communities, independent or assisted living
