Hospice
Our mission is deeply rooted in our core values, established in 1930 by the Sisters of St. Joseph of Orange: Dignity, Service, Excellence and Justice. We are committed to bringing these values to every aspect of care for patients living with life-limiting illnesses. We embrace a family-centered approach for the physical, emotional and spiritual care of patients. This special form of comfort care is provided to patients in their home, extended care or skilled nursing facility and, at times, in a hospital setting.
Services Offered
The Hospice multidisciplinary team provides the following services to patients and their families:
- Development of an individual plan of care
- Nursing visits to manage pain and other symptoms
- Assistance with personal care
- Telephone access to a Hospice nurse 24-hours a day, 7-days a week
- Counseling and emotional support
- Spiritual assessment and support
- Medications for pain and symptom management
- Durable Medical Equipment and medical supplies to ensure patient comfort and safety
- Bereavement support for family members for one year following the death of a loved one
- Perinatal Comfort Care Program
- Cardiac Program
- Dementia Program
Eligibility

Our Hospice Team
Our team of experienced Hospice professionals is dedicated to providing the best care possible in an atmosphere of compassion and dignity. Our team includes:
- Patient’s Attending Physician
Usually makes the referral to Hospice, writes orders for care and works with the Hospice team to achieve optimal pain and symptom management.
- Medical Director
Is actively involved in the development of the patient’s plan of care, provides consultation to the team members and the patient’s Attending Physician and makes home visits as needed.
- Nurse
Specially trained nurses that enhance the patient’s quality of life by providing pain and symptom management, support and education.
- Hospice Aide
Is available to provide bathing and personal care.
- Clinical Social Worker
Provides psychosocial support, counseling and advocacy to assist the patient and family with adjustment to the illness and end-of-life care planning.
- Chaplain
Assists with spiritual care, makes a connection with religious traditions and rituals if desired, and provides support for all faiths and denominations.
- Volunteer
Offers the patient and family companionship, respite and assistance with some activities of daily living.
- Registered Dietician
Offers consultation regarding diet and nutrition.
- Music and Massage Therapies
Are available on a limited basis to compliment the management of pain and other symptoms.
- Physical, Occupational & Speech Therapists
Provide consultation, assessment and instruction on a short-term basis.

Financial Services
For patients who have insurance, the coverage will vary depending on each individual plan. For patients who have Medicare or Medi-Cal, coordination of benefits is arranged through our agency. We will happily assist you to access Hospice benefits.
Referrals
Referrals to our Hospice program can come from any source. Did you know you can make your own referral? They can also come from hospitals, physicians, insurance case managers and families. For more information or to initiate the referral process, please call (714) 712-7110.
Accreditation
We are accredited by The Joint Commission. We are certified by Medicare and Medi-Cal, and we are California state licensed.
