Home is the place to be for the quickest recovery from an illness, injury or surgical procedure.

Within your familiar surroundings, our expert, skilled clinical staff will provide the best care possible, utilizing a holistic approach.  Working collaboratively with your physician, a plan of care will be developed to meet your unique needs.  Our compassionate and caring staff will assist you and your family to reach your optimal recovery goals.

The Joseph Health System Home Health Agency offers very specialized services for the community’s adult population who require intermittent care in their home for a variety of needs.  Our services include:

  • Congestive Heart Failure Program

  • Infusion Therapy, IV Antibiotics, TPN, Hydration
  • Central Line / Port-A-Cath Care

Our Home Health Team

Our experienced, multi-disciplinary team of certified and licensed staff work together to provide knowledgeable and comprehensive care.  Our team includes:

 

  • Registered Nurses & Licensed Vocational Nurses
    Provide clinical care in the comfort of your home.
  • Physical Therapists, Occupational Therapists & Speech Therapists
    Provide clinical care in the comfort of your home.
  • Certified Home Health Aides
    Make bath visits and provide personal care to assist with activities of daily living, all under the supervision of a Registered Nurse or Physical Therapist.
  • Wound Care Specialists
    Specially trained nurses work with your physicians to heal and care for your wounds.
  • Palliative Care Nurses
    Nurses proficient in pain management and end-of-life care provide a holistic approach to care, involving your physician, patient and family.
    *When requested, consultation and transition to the Hospice Program is an integral part of this service. 
  • Pharmacists
    Specialty Pharmacists provide for enteral and infusion needs of patients and are available to answer questions in relation to medication management.
     
  • Medical Social Worker
    Social workers are available to meet the psychosocial needs of our patients and work in conjunction with the rest of the team to assure that all aspects of care are addressed. 
  • Registered Dietician
    Offers consultation regarding diet and nutrition.
  • Triage On-Call Staff
    Registered Nurses are on-call 24/7 to respond to your calls, answer your questions and address your concerns.

We service the greater Orange County area.

Financial Services
For patients who have insurance, the coverage will vary depending on each individual plan.   For patients who have Medicare or Medi-Cal, coordination of benefits is arranged through our agency.  We will happily assist you with all insurance verification and authorization.

Referrals
Referrals to our Home Health program can come from any source.  Did you know you can make your own referral?  They can also come from hospitals, physicians, insurance case managers and families.  For more information or to initiate the referral process, please call (714) 712-7110.

We are accredited by The Joint Commission. We are certified by Medicare and Medi-Cal, and we are state licensed.
